I authorize the person listed above to have access to and use the personal and verification documents I previously submitted to H-Authentica Inc., including but not limited to my identification, selfie verification, void cheque, pay stubs, and other supporting documents required for account verification or financing purposes.

I understand that these documents may be used by H-Authentica Inc. to verify account activity, process orders, or confirm eligibility for financing or payment arrangements associated with my account.

I authorize the person listed above to place orders, communicate with H-Authentica Inc. on my behalf, and use the available credit limit or payment arrangements on my account, subject to H-Authentica Inc.’s policies.
